A Personal Emergency: The Hammer Incident

One of my most memorable experiences occurred when my childhood friend, Tom, was brought in for emergency treatment. Tom and I shared many fond memories from our childhood, and I had never imagined that he would one day seek urgent medical assistance due to a unique and unconventional medical accident. During an uneventful day, Tom was wheeled into the emergency room on a stretcher, carried in by his father, Pastor. To my surprise, he asked his father to leave the room, a request that raised eyebrows, considering their close bond.

Tom then confessed an unusual issue: his penis had become stuck in his zipper, and in an attempt to free himself, he had mistakenly inserted a large portion of a hammer handle into his penis. While such incidents are not uncommon, handling this case required a steady hand and quick thinking. After a thorough examination, a swift procedure was conducted, and a quick scan confirmed no internal bleeding. Tom was then escorted to see his waiting father, who was waiting anxiously but relieved to see his son's condition had stabilized.

Unfortunately, Tom kept the incident a secret, likely due to embarrassment, and even on Sunday, he did not disclose his true condition to his father. An Unexpected Journey: The Meningitis Emergency

On another occasion, I was the senior doctor on duty in the Emergency Department for the night. A senior nurse approached me with an urgent request to see the patient flow manager and senior matron, Rachel, who was experiencing a potentially serious allergic reaction. Despite the fact that it was a minor issue, the nurse was eager to continue her shift. I administered a course of antihistamines and advised her to return if her condition worsened.

However, the next night, Rachel called in sick. Just before 6:00 AM, the nurse receiving ambulatory patients came to find me. Rachel had been brought in by ambulance, and the nurse was worried. After assessing Rachel, I found her to be confused and disoriented, and she did not recognize me or recall her previous shift. Paramedics reported that Rachel had fallen during the night and had a high temperature and heart rate.

Fearing a potential brain-related issue, such as meningitis or encephalitis, I immediately placed her in resuscitation and started IV antibiotics and antivirals. An urgent CT scan was requested to evaluate her brain condition. Thankfully, the CT scan revealed no signs of bleeding or raised pressure, and Rachel’s symptoms improved over the next few days. This episode was particularly memorable and emotionally taxing, as I could not help but feel a sense of responsibility beyond my professional duties.

The nurse in resus later shared with me how it was the only time they had seen me appear worried or scared, as I had previously handled many critically ill patients. The allergic reaction, which Rachel experienced earlier that week, turned out to be a coincidence. The rash was a consequence of that allergic reaction, not the infection that ultimately required hospitalization.
